Welcome to West Wakefield Health & Wellbeing
West Wakefield is a GP federation of six neighbouring GP practices in Wakefield, West Yorkshire. As a group of doctors, practice managers and other healthcare professionals, we work together to provide the best possible healthcare for the local community. In total, we are responsible for around 64,000 patients.
Each member practice is an independent business but works collaboratively with its neighbours. GP practices up and down the country are forming federations and alliances and we chose to work this way locally because:
- It gives us the best chance of providing safe, sustainable healthcare locally in a time when GP practices are under significant pressures both financially and operationally.
- It means we can work together to deliver services at scale that are designed specifically for our local demographic both now and in the future.
- It allows us to pool our combined resources and knowledge to identify and meet the growing needs and expectations of our local population.
A new way of thinking, a new way of caring
We were one of the first areas in the country to take on the Prime Ministers Challenge Fund and in Autumn 2015, we were announced as an NHS vanguard site together with forty-nine other sites nationally.
Put simply, West Wakefield and its two partnering GP networks formed one of the very first collaborations in the country to start developing a new model of care known as a Multispecialty Community Provider (MCP) to look after our combined 152,000+ patient population.
