You may remember that up to April 2017, West Wakefield proudly hosted Wakefield's Multispecialty Community Provider (MCP) vanguard. Developing how we use social prescribing in the community was a key part of that work and we're pleased to see a tool that we helped to develop has now been endorsed by NICE.
Together with our partners in the Live Well Team from South West Yorkshire NHS Foundation Trust, the social prescribing tool consists of five sections based on the New Economics Foundation (NEF) Five Ways to Wellbeing and each section contains five questions that support the recommendations within NICE Guidance NG32.
The tool began its life with the help of guidance and support from the Rotherham Social Prescribing service and we are pleased to announce that the tool is now freely available for download below:
The user guides above have two sections. The first section is aimed at the social prescriber who will use the social prescribing tool in delivery. The second section is aimed at commissioners and providers of social prescribing services and can be utilised to establish a framework of key components, of which a social prescribing service should provide within communities.
